Clinical efficacy of a salicylic acid–containing gel on acne management and skin barrier function: A 21‐day prospective study.

Acne vulgaris is a common skin disorder driven by excess sebum, inflammation, and a weakened skin barrier. Although medical treatments continue to advance, growing consumer awareness has revealed persistent unmet needs in everyday skincare. This challenge underscores the importance of developing products that are both effective and gentle enough for daily use. Salicylic acid, a well-established beta-hydroxy acid (BHA), remains a key ingredient in acne care thanks to its ability to penetrate oily follicles, dissolve keratin buildup, reduce comedone formation, and provide mild anti-inflammatory benefits for lesions such as papules and pustules.
The current study by Liu Y, et al., evaluated the efficacy and tolerability of a salicylic-acid gel in reducing sebum, improving hydration, reinforcing the skin barrier, and easing acne symptoms over a 21-day period.
This single-center, prospective trial included 42 participants (37 women, 5 men; mean age 25.86± 6.69 years) with mild to moderate acne (IGA grades 2–3) and oily or combination skin. Participants used the gel twice daily for 21 days. Sebum output (Sebumeter SM815), trans-epidermal water loss (Tewameter TM Hex), and stratum corneum hydration (Corneometer CM825) were measured at several intervals. Acne severity was rated using the IGA scale, and participants also completed self-reported satisfaction questionnaires.
By the end of the 21-day follow-up, sebum production had decreased by 23.65% (p<0.05), and skin hydration had increased by 40.5% (p<0.05). TEWL was reduced by 49.26% (p<0.05), reflecting a strengthened skin barrier. Acne severity, measured by the IGA scale, improved by 23.81% (p<0.001). All participants reported satisfaction with the gel, noting better oil control, fewer breakouts, and smoother skin texture. Tolerability was high, with only 5% experiencing mild, temporary itching which resolved without intervention.
The salicylic acid–containing gel effectively reduces acne, controls sebum, boosts hydration, and strengthens the skin barrier, making it suitable for acne-prone, sensitive skin. Overall, it offers a balanced approach that supports both efficacy and user comfort.
