From Immune Dysregulation to Targeted Therapy: The Impact of Biologics in Atopic Dermatitis

Introduction

Atopic dermatitis (AD) is one of the most common chronic inflammatory skin disorders worldwide, affecting approximately 10–15% of children and 4–8% of adults. The disease is characterized by recurrent eczema, severe itching, skin barrier dysfunction, and immune dysregulation. AD significantly affects quality of life by causing sleep disturbances, emotional stress, reduced productivity, and social burden. In addition, more than half of patients with AD suffer from associated atopic conditions such as asthma, allergic rhinitis, allergic conjunctivitis, and food allergies.

Evolution of Biologic Therapy in AD

Before 2017, systemic treatment options for moderate-to-severe AD were limited to broad immunosuppressive agents such as cyclosporine and methotrexate. Although these therapies helped control disease activity, they were associated with significant toxicity, limited long-term effectiveness, and frequent treatment discontinuation.

Advances in understanding the immunological pathways involved in AD have led to the development of targeted biologic therapies. These biologics specifically inhibit cytokines responsible for inflammation and itch, thereby improving disease control with a more targeted safety profile. The currently approved biologics include:

  • Dupilumab – blocks IL-4 and IL-13 signaling through IL-4 receptor alpha inhibition
  • Tralokinumab – selectively targets IL-13
  • Lebrikizumab – selectively inhibits IL-13
  • Nemolizumab – targets IL-31, a key mediator of pruritus

Clinical Effectiveness of Biologics

Clinical trials and real-world evidence have demonstrated that biologics significantly reduce disease severity, itch intensity, and improve quality of life in patients with AD. Dupilumab has shown particularly strong and sustained long-term efficacy, with drug survival rates of approximately 80–90% over three years in adults. Similar benefits have also been reported in pediatric populations.

Tralokinumab and lebrikizumab have also shown significant improvement in clinical outcomes such as Eczema Area and Severity Index (EASI) scores, Investigator Global Assessment (IGA), and itch reduction. Nemolizumab appears especially effective in rapidly improving pruritus, with some patients reporting itch relief as early as the first week of therapy.

Adverse Events Associated With Biologics

Ocular Surface Disease

One of the most frequently reported adverse effects of biologic therapy, particularly dupilumab, is ocular surface disease (OSD), including conjunctivitis and blepharoconjunctivitis. Real-world studies suggest that OSD occurs more commonly than initially observed in clinical trials. Management usually includes artificial tears, topical tacrolimus, and ophthalmologic referral when required.

Skin Reactions and Facial Redness

Biologic-associated skin reactions such as psoriasiform eruptions, facial redness, rosacea-like dermatitis, and head-and-neck dermatitis have also been reported. These adverse effects are generally manageable with topical therapies, antifungal agents, or interval prolongation of biologic dosing.

Musculoskeletal Adverse Effects

Some patients experience musculoskeletal symptoms, including arthralgia, enthesitis, and inflammatory joint pain, during biologic therapy. Most cases respond to supportive treatment or dose adjustment, while only a small proportion require discontinuation of therapy.

Impact on Atopic Comorbidities

An important benefit of biologics, especially dupilumab, is their positive effect on associated atopic diseases. Studies have demonstrated improvements in asthma symptoms and reductions in food allergen-specific IgE levels during treatment. These findings suggest that biologics may not only control skin inflammation but also modify the broader atopic profile of patients.

Dose Tapering and Personalized Treatment

Real-world studies have shown that biologic dosing intervals, particularly for dupilumab, can often be safely extended once stable disease control is achieved. Many patients maintained remission with dosing every 3–4 weeks rather than the standard 2-week schedule. This individualized approach improves patient convenience, reduces adverse effects, and lowers healthcare costs.

Emerging Therapies and Future Directions

The article highlights promising future therapies targeting the OX40–OX40L signaling pathway, including rocatinlimab and amlitelimab. These therapies modulate multiple inflammatory pathways and have demonstrated durable responses even after treatment discontinuation. Such findings suggest the possibility of long-term disease modification in AD.

Additionally, biologics have shown beneficial effects on skin barrier repair, microbial diversity, and reduction of Staphylococcus aureus colonization, indicating that these therapies address underlying disease mechanisms rather than merely suppressing symptoms.

Biologic therapies have revolutionized the management of moderate-to-severe atopic dermatitis by providing targeted, effective, and relatively safe treatment options. Dupilumab, tralokinumab, lebrikizumab, and nemolizumab have demonstrated substantial clinical benefits, including reductions in disease severity, itch control, and improved quality-of-life. Despite these advances, unmet needs persist among patients with inadequate responses or treatment-limiting adverse effects. Future research focusing on biomarker-driven therapy selection, treatment tapering, and disease modification may further optimize long-term management of AD.
